Removing nasal valve obstruction in peak nasal inspiratory flow measurement

Martyn L. Barnes, MBBS
Brian J. Lipworth, MD

Asthma and Allergy Research Group, Department of Medicine and Therapeutics, Ninewells Hospital and Perth Royal Infirmary, University of Dundee, Dundee, Scotland.

Peak nasal inspiratory flow (PNIF) measurements are used to evaluate nasal obstruction and as a surrogate for disease activity in allergic rhinitis and other nasal complaints. This measurement can give erroneous results when forced inspiration leads to nasal valve collapse.

To determine the effects of 2 different nasal stents (Sinus Cones and Nozovent) on valve collapse and repeatability of PNIF measurements.

Repeated measurements of PNIF were obtained in 74 individuals with and without 2 different nasal stents: Sinus Cones and Nozovent.

The mean (95% confidence interval) improvement in PNIF was 1.7 L/min (−2.4 to 5.8 L/min; P = .42) with Nozovents and 25.4 L/min (11.4 to 39.4 L/min; P = .001) with Sinus Cones. The PNIF coefficient of variation for repeatability was 11.6% without stents, 16.0% using Nozovents, and 10.4% using Sinus Cones.

Sinus Cones, but not Nozovents, significantly improved PNIF. Repeatability of PNIF measurements was worse with Nozovents and only marginally improved with Sinus Cones.

Follow our patented sizing guide to determine your most suitable size.

Measure your thumb as shown.

Measure your thumb as shown.

  1. Place your thumb on a ruler, fingerprint side down with the left edge of the thumb on the beginning of the ruler or 1 inch mark.
(In the image shown above, the left side of the thumb is placed flush with the 1 inch mark on the ruler.)
  2. Measure across the widest part of the thumb as indicated by the black arrows above.
(In the image above, the right side of the thumb measures about 7/8″, or less than one inch.)
  3. Use this gauge to select your size.
(In the image above, based on a measure of 7/8″ and referencing the chart below, this thumb measures to a medium size.)

If your thumb measures:

3/4″ (.75 of an inch) or less, select the size small.

If your thumb measures more than 3/4″ to 1″ (.75 of an inch to one inch), select the size medium.

If your thumb measures more than 1″ (more than one inch), select the size large.

If you are right on the line between two sizes, we suggest ordering both sizes, trying them and then returning the ones that are not the correct size. Please see our guarantee for details.

PLEASE NOTE: sometimes nasal airways are not the same size and two sizes are necessary. So be sure to try each size in each airway.

SINUS CONES are made connected to one another.
To achieve just the right fit and comfort, they may be separated and worn individually.

Custom Sizing for Deviated Septum and Nasal Airway Collapse

Mixing two different sizes may be needed for deviated septum or nasal valve collapse, if one nasal airway or nostril is smaller or narrower than the other.

If this is your situation, you will find it more suitable to use two different sizes Cones to treat this problem. Simply determine the size that you measure to normally, as indicated above, and then select that size for your larger nostril, and the next smaller size for your other smaller or narrower nostril.

Purchase the two sizes, and use one size in one nostril and the other size in the other nostril. Once again, swivel for your most comfortable fit and best angle to maximize flow volume.

How to Choose Your Most Suitable Squeezieness-Rating: Soft, Softer, or Softest

Every person is slightly different when it involves the softness that they prefer.

Generally the softest, clear, Max-Air Nose Cones are preferred, especially for mild deviated septum, for combination therapy with oral appliance or CPAP for mild sleep apnea, to stop snoring, and to relieve night time dry mouth.

The original, softer, white Sinus Cones may be best for those nights when nasal allergy, sinus or cold congestion is severe and extra nasal airway support is necessary, when snoring is loud and pervasive, or when nasal obstruction from a collapsed side-wall is present.

The new, soft Max-Air Nose Cones Performance are designed to stay put so that you don’t have to; as such their fit and feel is a little more dynamic to insure a secure fit no matter how you plan to wear them.

For some individuals this is perfect for running faster than a speeding bullet or leaping tall buildings in a single bound; however it is too dynamic a feel for their sleep, where they prefer the Sinus Cones or regular Max-Air Nose Cones. That said for other individuals the new, soft Max-Air Nose Cones Performance are just right for day and sleep wear.

If you are undecided, why not order both. Try them out, decide which is more suitable for your situation, and return the other set for a free exchange.

Helps to avoid mouth breathing in my sleep, which has led to fewer mouth sores and better sleep, but the only downside is that using them multiple nights in a row leads to my nose being sore.

Overall very helpful, but not comfortable enough to wear every night.

    • Nose News

Hi Jeremy – thank you for posting your review. There should never be any discomfort when using our products. If there is, that indicates that the size is too large, and a smaller size is warranted. Please reach out to our tech support for assistance in determining a more appropriate size and making an exchange.

Husband has had mouth cancer surgery and had most of his top palate removed. Found his nose collapses in to where he can’t breathe. Have tried many different brands of apparatuses and THIS is the best out there. He can now breathe again!!! He even wears some for work!! If you have a collapsing nose, GET THESE!!! YOU WON’T BE SORRY!! BEST CONES EVER in my opinion!!!

  3. Debora (verified owner)

My ENT recommended these Sinus Cones but I couldn’t find them locally so I tried another brand. The other brand were rough, hurt and you couldn’t separate them. The Sinus Cones on the other hand are soft, fit comfortably, and you don’t have to wear them both. Much better sleep! And dry mouth is going away.

  4. Joseph P, MD

Being able to comfortably breathe through one’s nose, at rest or with exertion, is an underappreciated component of physical health and overall well-being. Chronic mouth inhalation is associated with negative health consequences; and it is no coincidence that most yoga breathing exercises (Pranayama) require conscious, controlled nasal inhalations across a broad range of speed and forcefulness.

I had difficulty with nasal breathing right from the start. Allergies to cat dander and seasonal pollen made things worse. In medical residency I acquired a severe latex allergy, leading to chronic rhinitis, obstruction and episodic bacterial sinusitis. At this point, topical and oral medications were of little help. Because the problem interfered with my clinical duties, I presented myself for an ENT surgery consultation. After imaging and examination, I was told I sustained a nasal fracture from a traumatic birth, ultimately resulting in a badly deviated nasal septum, which now required surgery.

The septoplasty/turbinectomy helped a little, but also weakened structures that normally keep the nasal air channel open, especially during rapid or forceful inhalation. The problem now seemed to interfere with sleep even more than before. Symptoms could occur even by merely laying down to rest. The condition noticeably limited my exercise capacity and efficiency.

I tried nasal strips intermittently for years, with some success. However they caused skin inflammation when used more than two or three times a week. I also tried most of the many other insertable devices on the market. All were unsatisfactory. None came anywhere close to the relief I experienced after discovering Max-Air products.

Before Max-Air, I woke up multiple times a night, usually with a dry mouth, and often with a headache and anxiety. This was in spite of near-optimal sleep hygiene. With Max-Air, and slightly elevating the head of my bed, nighttime symptoms virtually disappeared. I now reliably enjoy restful, restorative sleep, and have better energy and focus throughout the day. I also use Max-Air frequently when cycling or playing basketball.

I recently tried using Max-Air under a bandana while cycling. They performed well but would occasionally slip down, requiring me to push them back up. I then tried them under a N95 mask, which caused less direct pressure on the nose. They were more stable, noticeably reducing my obstruction symptoms. So I wondered if these would be helpful to others experiencing obstruction while wearing a mask.

Comfort, performance and durability make Max-Air products a first choice for me. They are worth trying for anyone with nasal inspiratory obstruction, especially while wearing a face mask. One thing is for sure, Max-Air products are true to their name, providing Maximal Airflow through the nose for various activities and under many conditions and circumstances.

Thank you to Sanostec and Max-Air.

  5. Cathie Ruggiero

I was amazed at the difference I felt immediately. I slept all night, had no sinus headache (most always do), no dry mouth, and, a huge reduction in post nasal drip. I have much more energy during the day now. They have come out while I’m sleeping so I think I need to get a medium.
